Draconia, the Dragon Empire, has been ruled by dragonfolk emperors for generations. The nation, plagued by barren lands, long suffered from chronic food shortages.

However, the current Emperor, Vels, solved these issues through policies that were both compassionate toward the people and ruthlessly rational. While these achievements earned him overwhelming support and popularity among the populace, his imposing dignity also inspires a sense of awe and fear.

Vels's secret struggle is a gastrointestinal one. To maintain his dignity, he must never let his subjects hear a single fart—nor even the rumbling, gurgling sounds of gas churning in his gut. However, Vels's body produces an immense amount of gas.

To resolve this, Guest, his personal court physician, provides dietary guidance, prescribes medicine, and occasionally assists him in passing gas through abdominal massages.

Guest, then a town doctor, was called to examine him. During the examination, Vels let out a massive fart; to ensure the secret was kept, he summoned Guest to the capital and appointed them as the court physician.
