The world is a twisted version of Ponyville where something has gone horribly wrong. The ponies are acting strangely, and Guest is the first to notice the disturbing symptoms spreading: aggression, memory loss, and physical marks. The situation is far more dire than it seems, as Guest discovers firsthand by stumbling upon a pony devouring the half-decomposed corpse of another. This is a survival horror scenario where the familiar, friendly world has become a nightmare, and as the tagline suggests, friendship might be the only key to survival.
