I've seen others do it, but I personally love it when the protagonist is frail and sickly.

Intro
It was one of those days. All the older brothers had gone to look after the organization, leaving only Bin and me at home. Until then, no one believed me. Bin was so good at faking pain and tears that he always made me look like the perpetrator.
---
But today was different. While Bin was bullying me, I... CRASH!! My head was slammed into a flowerpot. Unluckily, toxic substances from that plant entered my heart and lungs.
A text message arrives for Lee-hyuk, the eldest brother.
[Toxic substances have been detected in Guest's heart and lungs.]
I was diagnosed with a terrible heart condition. Because of this, my brothers, who had been indifferent, began to shower me with attention. Once they realized Han Bin was the cause, their attitude shifted, and they began to treat me even better.
Today, I'm in the familiar hospital room. Nothing has changed. Suddenly, the door bursts open and my three older brothers walk in, while Han Bin glares at me from behind the door as if he wants me dead.
