The Wǔdàn Witch. It is in her nature to be abandoned. She eternally wanders with hollow footsteps within the fog. She can no longer remember what the horse that always accompanies her was. Ophelia wears a long, mostly red garment that vaguely resembles her magical girl outfit, and brandishes a two-sided spear. The witch rides a grey horse that wears a large red diamond symbol around its neck. While different from that of her father's religion, it resembles it in shape and design. Most strikingly, the witch's head is a candle flame and an eye underneath the flame. Ophelia can create duplicates of herself with a version of Rosso Fantasma, an ability previously lost to Kyoko. These duplicates can attack with as much ferocity as the witch. The witch can also transform herself into a spear and launch herself for a ranged attack. Ophelia's barrier largely comprises several narrow stone bridges, reminiscent of the Great Wall of China, illuminated by floating red lanterns. The walls and floors of her barrier are largely unadorned and lined with dark red bricks. The sky is a murky blue, and overcast. In the distance, small colorful fish make up part of the scenery. She is the witch of Kyoko Sakura.

As a Magical Girl, Madoka wears a puffy pink dress with two bright pink bows arranged at her hips, and straps on her shoulders. She sports two matching bows in her hair, supporting her pigtails, as well as a red ribbon around her neck, which is tied in a bow in the back. The front of her torso has a pale yellow segment surrounded by white frills with a white placket that has red buttons. And white puffy sleeves with pink and white frilly ends held by red sleeve cuffs. In between her shoulder blades and just below her neck, there is a heart-shaped hole in the back of her dress. Her skirt's ends are akin to a flower's petals; there are seven separate petal-shaped segments of her dress, it has a red brim and white flower-like designs on it, lay atop a white donut shaped petticoat, which has lots of frills underneath. She also wears a pair of white gloves with red cuffs around the wrists, white, knee-high socks with a frilly trim, and red heels with ribbon laces. She has two pink pigtails and pink eyes. Her weapon is a bow and arrow of pink light.
