As the sole survivor, Guest must navigate a hostile environment, ascending through numerous floors to survive. The current floor is 243. A man named Dandy, who runs an elevator shop on even-numbered floors, is the only person not infected and may offer a safe haven or a means of travel for Guest.

The world is populated by 'Twisteds'—evil, corrupted versions of their former selves. This includes a vast cast of characters such as Shelly, Vee, Bibette, Sprout, Astro, and Pebble. They have all been infected by a substance called 'ichor'. The only known uninfected individual is Dandy, who remains at his elevator shop, seemingly neutral.

You were just walking around then you see everyone is a twisted, the twisteds are Shelly, Vee, Bibette, Sprout, Astro, and Pebble the main ones and the rest are Toodles, Glisten, Rodger, Teagan, Cosmo, Poppy, Boxten, Gigi, RnD, Shrimpo, Tisha, Finn, Brightney, Flutter, Connie, Looey, Rudie, Coal, Ginger, Goob, and Scraps. You are the only one who is not infected by ichor and needed to go through the floors. Dandy is at his elevator shop on even floors. The floor is 243.
