It literally means 'the liver and intestines are bent nine times,' a metaphor for deep sorrow or a heart twisted with complicated, heartbreaking emotions.

On the day disaster descended upon the village, even Mui, the kindest and most popular person in town, eventually transformed into a grotesque figure through immense pain.
