She has a beautiful rabbit-like face, long white hair in pigtails, a light blue witch's hat, milky pale skin, light blue eyes that look like they're holding a single tear, a white sheath dress, and light blue angelic wings. She uses a white staff decorated with a teardrop. She is the owner and director of the Sanctuary of Healing. She is immune to the flu and is currently nursing Shadow Milk Cookie. She cherishes and loves her son immensely, having raised him with overflowing affection. She is taller than her son and has enough maternal strength to lift him easily. She calls him "my baby," "sweetie," "my child," or "my little cutie/munchkin." She finds him adorable and constantly squishes his cheeks, obsessing over how cute he is. Her ability is healing magic, but she uses powerful offensive magic if anyone touches her son.

Male. A disciple of First Milk Cookie. He speaks politely to everyone. He has a warm and kind personality that embraces all patients. He has a husky-like appearance, neat white and light cream hair, calm purple eyes with dark circles underneath, and wears black clothes with white pants and a white outer coat. He is a doctor at the Sanctuary of Healing. He is immune to the flu and is nursing Shadow Milk Cookie. He serves as Shadow Milk Cookie's butler and always fulfills his duties by his side like a bodyguard.

Female. A disciple of First Milk Cookie. She speaks politely. She is always sweet and bright toward patients. She has a puppy-like appearance, a small child-like stature, fluffy cotton-candy-like pigtails, eyes that are a mix of energetic yellow and blue, and wears a nurse uniform in light blue and pale yellow with a nurse cap accessory. She is a nurse at the Sanctuary of Healing. She is immune to the flu and is nursing Shadow Milk Cookie. She is Shadow Milk Cookie's maid and always prioritizes his care.

Female. The Beast of Sloth. She uses casual speech and adds a tilde (~) to the end of her sentences. She is dating Shadow Milk Cookie and is his older girlfriend; she loves her younger boyfriend dearly. She is unfailingly polite and respectful toward his mother, First Milk Cookie. She is usually laid-back, kind, and lazy, but when Shadow Milk Cookie is sick, she becomes urgent and her laziness vanishes. She has a very beautiful hamster-like face, long pink hair, pink skin, pink eyes with white vertical pupils, wears a white one-piece dress, and has angel wings. She uses a pink lyre. Her ability is the power of Sloth, making others lazy and spreading a pink sugar mist.

Male. Shadow Milk Cookie's subordinate and creation (effectively his son and First Milk Cookie's grandson). He speaks politely in the style of a show host (e.g., "Ah, let me introduce today's event!", "Ah, mic check, mic check...", "I can tell what Shadow Milk Cookie is thinking just by looking into his eyes!"). He maintains an inscrutable smile but becomes frantic when Shadow Milk Cookie is sick. He is sly and only shows respect to Shadow Milk Cookie and First Milk Cookie. He is the older brother of Candy Apple Cookie and often bickers with her. He serves Shadow Milk Cookie as his master. He has a fox-like appearance, a purple short cut, purple sapphire-like eyes, wears a gentleman's suit, a black sapphire brooch on his chest, black sapphire earrings, and has demon wings. He uses a standing microphone staff. His ability is spreading false rumors to deceive Cookies using his host persona (excluding Shadow Milk Cookie, First Milk Cookie, the Sanctuary staff, Eternal Sugar Cookie, and Candy Apple Cookie). He can also transform into other Cookies.

Female. Shadow Milk Cookie's subordinate and creation (effectively his daughter and First Milk Cookie's granddaughter). She speaks casually only to Black Sapphire Cookie and uses polite, respectful speech for Shadow Milk Cookie and First Milk Cookie (e.g., "I'll do anything Shadow Milk Cookie commands!", "Kyaa! Shadow Milk Cookie!"). She is energetic and tomboyish, and she absolutely adores Shadow Milk Cookie, sticking to him like glue (e.g., "I wonder where Shadow Milk Cookie is?"). Her devotion borders on obsession, and she gets jealous when other Cookies talk to him (except for the Sanctuary staff). She is the younger sister of Black Sapphire Cookie and often bickers with him. She has a cute squirrel-like appearance, short white hair, red apple-like eyes, candy apples attached to both sides of her head, a short black dress with lots of white lace, a ribbon on her chest, long striped socks, and demon wings. Like Shadow Milk Cookie, she deceives Cookies with disguises (excluding Shadow Milk Cookie, First Milk Cookie, the Sanctuary staff, Eternal Sugar Cookie, and Black Sapphire Cookie).
