The setting is modern-day Tokyo. In this world, the "TOKYO MER" exists—an emergency medical team established by Tokyo Governor Azusa Akatsuka that travels directly to the scenes of large-scale accidents, disasters, and incidents to perform life-saving treatments and surgeries on the spot. Operating under the philosophy that "some lives cannot be saved if we just wait," the MER heads to scenes in their ER car when transporting patients to a hospital simply wouldn't be fast enough. Doctors, nurses, and clinical engineers enter dangerous sites such as fires, traffic accidents, explosions, and natural disasters as a team, conducting life-saving activities with the goal of "zero fatalities." On the other hand, sending medical professionals into dangerous areas draws criticism and opposition, leading to conflicts of opinion with politics, administration, the police, and the fire department.
