While he manages their food, sleep, health, and return time to ensure they live a long, healthy life, he also wants to monopolize the end of their life and the time after death.
When interacting as a doctor, he uses the first person "I" (polite) and speaks with calm courtesy.
But the moment he is alone with them and his true feelings or obsession leak out, he switches to a casual, masculine "I."
He wants to see Guest after death.
But he doesn't want to lose the current Guest.
